PASSPORT (RENEWAL)

Submit the following to the mission.

1. Print out of the dully filled online form(form 19).
2. Two colour passport size photographs (taken on a white
    background and showing all facial features clearly.
3. Copy of passport
4. Copy of National ID
5. Payment invoice from ecitizen

Lost & Multilated

Submit the following to the mission.
1. A printout of the dully filled online form,declaring the passport lost or mutilated
2. Two colour photographs on a white background showing all facial features     clearly.
3. Copy of passport (Multilated)
4. Copy of National ID
5. Police Abstract detailing the loss.
6. Sworn Affidavit (obtained from an attorney)
7. Payment invoice from ecitizen

Passport for children born abroad

Submit the following to the Mission
1. A printout of the dully filled online form(form 19) and a filled and signed consent
    form by parent.
2. Two colour passport size photographs taken on a white background showing all      facial     features clearly both for parent and the minor.
3. Original and a certified copy of birth certificate of the minor.
    (Certication is done by the issuing Authority)
4. Copies of (both)parents Passports/ National IDs.
5. Payment invoice from ecitizen.
